I certify that I am submitting a CD to which I hold All Rights, and can authorize airplay exempt from all royalty or licensing fees.

In return for valuable consideration in the form of promotional exposure, the above named Artist consents to the broadcast of the MP3 file or tracks from the CD named above on BluesRoot Radio.

Artist as the sole owner(s) of ALL RIGHTS in and to the Recording, hereby grants unlimited, non-exclusive streaming internet air play on BluesRoot radio. Artist understands that there will be no compensation for any streaming broadcasts, bios, or promotional materials related to the recordings or the artist..

Artist hereby releases, and discharges BluesRoot Entertainment from any and all liability arising out of or in connection with the use of the submitted recordings.

Submission of this completed form shall constitute acceptance of all terms above. While we reserve the right to accept or reject any material without cause or explanation, we will always try to respond via email with acknowledgement of receipt and a quick decision.
